About Us
Pact Home Health Care provides 245D services to people with intellectual and developmental disabilities and their families in their homes. We offer Adult Companion, Homemaker, Individualized home supports with and without training, Night Supervision, Personal Support, and supported living services by providing a highly trained direct support professional staff of caring individuals.
Person-Centered Services Provided
Individualized Home Supports
Services for people who live in their own home or family home that provide support focused on community integration, life skills and person centered goals and outcomes.
Personal Support
Services provided in the person’s home or community to increase and encourage independence, help nurture relationships and assist with activities of daily living.
Homemaking
Homemaker/cleaning/assistance with activities of daily living (ADLs)
Supported for Adults
A real-time, two-way communication between the provider and the persons.
Night Supervision
Overnight assistance and monitoring provided by an awake staff in the person’s home
